Welcome to the SRSLY Low Carb family! This sign up form is only meant for Ketogenic patients registered with their respective NHS Trust. Signing up will be done by the responsible NHS dietician the patients are registered with.

Once they are signed up, your patient will receive a link via email and text message to confirm they have been registered under the scheme. Only patients registered under this scheme will be entitled to the discount code and it is tied to their email address.
